Accommodation Form

All players participating in Wheelchair Lacrosse USA (WLUSA) events are required to wear full protective equipment, including a helmet, gloves, arm pads, and SEI-certified chest protection. Additionally, all players are required to have BOTH a lap strap and foot strap engaged and taut at all times to protect against injury and to prevent an unfair advantage.

Per WLUSA rules, reasonable accommodations can be made for players who have special needs or unique and extenuating circumstances as long as they do not fundamentally alter the sport, heighten the risk of injury to the athlete/others, or place opponents at a disadvantage.

To request an accommodation, please fill out the form below with all necessary information. WLUSA must have this form on file before competition in order to provide any accommodations.
